Tattoo Waiver - Blacksheep Portraits

I understand the inherent risks of receiving a tattoo and agree to the conditions above. I am of legal age (18+), signing in full competency, without coercion or under duress, and not under the influence of drugs and/or alcohol.

I understand that a tattoo is a permanent change to my appearance.

I understand being tattooed is uncomfortable and/or painful, and will inform my artist if my discomfort becomes intolerable.

I understand that the artist can cease the tattooing process at any time should my behaviour, pain tolerance, health, or any other concerns affect my ability to be tattooed.

I have been fully informed of the inherent risks associated with getting a tattoo, known and unknown, and that these risks can lead to injury not limited to infection and scarring, or allergic reactions to tattoo pigment or materials used during my tattoo (e.g. latex, soap).

TO WAIVE AND RELEASE to the fullest extent permitted by law each of the Artist and the Tattoo Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from my tattoo, whether caused by the negligence or fault of either the Artist or the Tattoo Studio, or otherwise.

My artist has provided me with full opportunity to ask and answer any questions and concerns about the tattoo process and they have been answered to my satisfaction.

I have approved the design of my tattoo and understand the artist is not responsible for the meaning or spelling of the symbol or text that I have approved as part of my design.

I understand there may be variations between the design and colours I have selected and approved and what the tattoo looks like when it is applied to my skin.

The Artist and the Tattoo Studio have given me instructions on the care of my tattoo while it's healing, and I understand them and will follow them.

I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me. If any touch-up work to the tattoo is needed due to my own negligence, I agree that the work will be done at my own expense.

I understand my tattoo will change over time; colours and clarity will fade over time due to the natural dispersion of pigment under the skin and sun exposure.

I understand that tattoos on FINGERS, TOPS OF HANDS, PALMS, AND FEET will fade and fall out unevenly due to the nature of the skin in these areas, and not due to the application by the artist. Touch ups to these areas are not provided for free.

I am not under the influence of any drugs or alcohol. I will not partake in any drugs or alcohol during the process of being tattooed (i.e. on breaks).

I confirm that I do not have a cough, fever, chills, shortness of breath, or any other symptoms related to illness or communicable diseases.

I am not pregnant or breastfeeding.

I do not have any of the following conditions, and if I do, I have disclosed them to the artist and assumed the risks associated with proceeding with my tattoo: diabetes, epilepsy, hemophilia, heart conditions, or bloodborne illnesses such as HIV, hepatitis, or AIDS.

I am not the recipient of an organ or bone marrow transplant, and if I am, I am taking the preventative antibiotics/medication required by my doctor prior to any invasive procedure like tattooing.

I agree to reimburse each of the Artist and the Tattoo Studio for any attorneys' fees and costs incurred in any legal action I bring against either the Artist or the Tattoo Studio and in which either the Artist or the Tattoo Studio is the prevailing party. I agree that the courts of British Columbia in Canada shall have personal jurisdiction and venue over me and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of or related to this agreement.

I consent to a full release of images taken of my tattoo and allow Blacksheep Portraits to use any of the images captured.

I do not consent to images taken of my tattoo to be used by Blacksheep Portraits.

I acknowledge that I have been given adequate opportunity to read and understand this document, that any and all of my questions have been answered, that it was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and the Tattoo Studio.

I agree that the personal information below is true and complete. I agree to complete ALL required fields without omitting any information (full name, full address, legal age, and complete contact information). I understand that the tattoo artist has the right to refuse service if any of the personal information provided is incomplete or untrue.
